What It Is

Enhanced Case Management (ECM) gives your family a Lead Care Manager: one person who knows your situation, keeps every provider talking to each other, and goes with you into the parts of the system that are hardest to move.

They meet you at home, at a shelter, at school, at the clinic, or wherever is easiest for you. Most families see their Lead Care Manager once or twice a month.

More than one person in a home can enroll. A child, a parent, a grandparent or an older sibling can each have their own Lead Care Manager and their own goals. 

Who Is It For

ECM is for Medi-Cal members whose health needs are tangled up with everything else going on. You may be a good fit if someone in your household is:
  • without stable housing

  • going to the emergency room often

  • living with serious mental health or substance abuse needs

  • involved with child welfare or foster care, up to age 26
